Incident Description
On Wednesday January 24, 2024 at approximately 12:30pm an RTD light rail train, operating a southbound “E” Line service from Union Station to Ridgegate Parkway, derailed on a “Hi-Rail” access pad located immediately north of Hampden Ave and approx. 1/2 mile north of Southmoor Station. The train consisted of 3x Siemens SD-100 units with light rail vehicle (LRV) #125 leading. The LRV began shedding wheel and tire components between Louisiana Pearl Station and University Station and the operator noticed “performance issues”. By Colorado Station, wheel and tire components were being shed at an increasing rate. The operator contacted Light Rail Control asking about previous reports of issues with that specific train from other operators and was told there were none. The train continued southbound from Colorado to Yale Station with “significant performance issues” and just south of Yale Station, experienced catastrophic failure of one wheel / tire assembly with another tire also being affected. The train finally came to a stop when it struck the “hi-rail” access grad crossing pad at Hampden Ave. There were no injuries. There was significant damage to alignment infrastructure, rolling stock and the wheel structure of the LRV.
Initial Response
As the incident constituted “A Derailment (Mainline or yard)” under 49 CFR Part 674.33, it was reported by RTD to the Colorado PUC and the Federal Transit Administration (FTA) within two hours of happening.
RTD initially described the incident as a “disabled train” in its service alerts.
Train Recovery
Initial efforts to re-rail the leading LRV proved difficult due to the constrained nature of the site and snowfall. The LRV was finally re-railed on Monday January 29th and towed to Mariposa Yard.
Investigation Timeline
May 29th, 2024 – Special Board Meeting (Executive Session)
On May 29th, 2024, a special board meeting was held with an executive session for Directors to receive legal advice “regarding the January 24, 2024 E Line incident north of Southmoor Station”.

Jun 6th, 2024 – Colorado PUC Accident and CAP Report Summaries Reveal a Wheel / Tire Failure is the Cause
On Jun 6th, 2024,Colorado PUC staff publish accident and CAP report summaries which give some basic details of the incident.
We learn that “the derailment was due to the LRV shedding its wheel / tire components and eventually experiencing a catastrophic failure of the wheel / tire assembly causing it to derail.”
An accident investigation report, A1-01242024, will remain confidential due to Colorado Revised Statute § 40-18-104.
A Corrective Action Plan, CAP01-01242024, has not been released to the pubic.
Jun 20th, 2024 – Colorado PUC Approves the Accident Investigation Report A1 and Corrective Action Plan CAP01
On Jun 20th, 2024, the Colorado PUC issued Decisions No. C24-0425 and C24-0429 which approved RTD’s Accident Investigation Report A1 and Corrective Action Plan CAP01 along with other unrelated reports and CAPs.
